Every tier opens behind the waitlist, and nothing here is billable today. Prices are posted in full anyway, because you should not have to sit through a sales call to learn a number. The exam finder is free and live now.
What the countdown looks like
90 days out
Baseline. Home readings, labs where needed, meds reviewed, a target set against the FMCSA staging table.
60 days out
Adjust. Check-ins from the cab, treatment tuned while there is still time for it to work.
30 days out
Confirm. You and your clinician watch the same numbers the examiner will see, so nothing on exam day is news.

BP, lipids and diabetes care

Focused tiers
Sleep pathway
$89/mo
Home sleep test referral, treatment support if you are diagnosed, and help sticking with it. No scare tactics: FMCSA regulations do not specifically address sleep apnea and there is no BMI trigger, so testing is your examiner's judgment (FMCSA). We work on sleep because tired driving is dangerous and treatment changes how you feel.
Smoking cessation

Quit support with medication where appropriate. In the CDC NIOSH survey of 1,670 long-haul drivers, 51% were current smokers, against 19% of US working adults.
TRT
$149/mo
For lab-confirmed hypogonadism only, and we say so up front. Testosterone is a Schedule III controlled substance. Remote prescribing rides on federal telemedicine rules that can change, and some states add their own. When remote is not allowed, this tier requires an in-person visit we help you schedule.
Mental health

Framed around sleep and stress, because that is what the road breaks first. Plain conversations with licensed clinicians, on your schedule, from wherever you parked.

Weekly billing, on every plan
Per-mile pay lands weekly and unevenly. Every plan above can split into four weekly charges per 28-day cycle instead of one monthly hit. Same totals as posted, no fee for splitting, cancel rules identical either way.
Straight answers
Is this the DOT physical?
No. The exam is hands-on and must be done in person by an examiner listed on the FMCSA National Registry, and FMCSA has published no telehealth pathway for it. Roadworthy is the care between exams. For the exam itself, use the free finder.
When do programs open?
When the clinical network and state coverage are ready, not before. The waitlist decides onboarding order, and telling us your exam month lets us time your onboarding to your countdown.
What is the 3-month card everyone dreads?
A one-time 3-month certificate for a systolic reading of 160-179 or a diastolic of 100-109, meant to get treatment started. Come back at or under 140/90 and the examiner can certify you for a year. It cannot be repeated, which is why those 90 days matter so much.
Do you take insurance?
No. Planned launch pricing is cash-pay, posted in full, with weekly billing because driver pay runs weekly. If that changes, this page will say so.
What about compounded GLP-1s?
Straight answer: FDA declared the tirzepatide shortage resolved in October 2024 and the semaglutide shortage resolved in February 2025, which narrowed what compounding pharmacies may produce. What we can offer at launch depends on the FDA status current at that time. We will state plainly what is available when programs open, and we will not sell workarounds.
Can you promise the exam goes my way?
No, and walk away from anyone who says yes. The certificate decision belongs to your examiner on exam day. Our job is the 90 days before the exam, not the 15 minutes inside it.
Which states will you cover first?
Coverage keys to where you are at visit time, because licensure follows the patient and this patient population crosses state lines for a living. Home states on the waitlist set the buildout order.
